Office Policy

Our primary goal is to restore and maintain your health. If we do not believe your condition will respond to chiropractic care, we will not accept you as a patient but will refer you to another healthcare provider, if appropriate.

Appointments

We request at least 24 hours notice in order to reschedule your appointment.

After Hours

If you have a chiropractic emergency after hours, we reserve the right to charge an after-hours fee of up the $50.00, payable at time of service. This fee is not billed to nor paid by your insurance (if applicable). It applies in addition to and separately from any fees for chiropractic adjustments, co-payments or payments towards your deductible.

Patient Messaging Services

By supplying us with your home phone number, mobile phone number, email address and any other personal contact information, you authorize us to employ a third-party automated outreach and messaging system to use your personal information and to transmit such information as the time and place of your appointment, a missed appointment and other communications.

You also authorize your healthcare provider to disclose to third parties, who may intercept these messages, limited protected health information regarding your healthcare events. You consent to receiving multiple messages per day from the automated outreach and messaging system when necessary.

Payment

Insurance Coverage

Insurance is a contract between the insurance company and yourself and the insurance company makes the final determination of your eligibility and benefits.

You are personally responsible for payment, but as a service to you, we can bill your insurance for your visits when you sign this form. We will prepare reasonable and necessary reports and forms to assist you in receiving reimbursement from your insurance company and payments made will be credited to your account.

If your insurance company is not contracted (“out-of-network”) with us, you agree to pay any portion of the charges not covered by them.

Co-payments, payments towards deductible and payments for services not covered by your insurance are due at time of service.

For Medicare patients, payment for exams and re-exams required by Medicare and all services not covered by Medicare is due at time of service.

When care is suspended or terminated, any fees for services rendered to you will be immediately due and payable.

No Insurance Coverage

Payment is due at time of service. A time-of-service discount will be extended if services are paid in full on the day that treatment was received.

Aged Balances

Balances 30 days and more past due will accrue a $5.00/1.5% late charge per month, whichever is greater. If your account is delinquent past 60 days without a financial arrangement having been made with the office staff, you may be turned over to the collection agency used by the Alma Chiropractic Center LLC. If you have been turned over to collections and seek treatment at the Alma Chiropractic Center, LLC all payments are due in cash before time of service and if insured charges will be submitted to your insurance one time only.
